Custom Graphics Worksheet
Use this form to open a new job with Dance Art FX, start the process of ordering your custom t-shirts, and place your deposit. Tell us which design you want to use, the date of your event, by when you will need your shirts, what kind of sample(s) you want, the exact spelling of words in your art. This form also places your order for Papergoods, telling us how many posters, tickets, program covers, etc., you will need. Please do include when you need each item delivered. At this stage, you can estimate quantities if exact figures are not known. You will be mailed a Papergoods Order Confirmation to confirm all details of your order and to make corrections before the order is produced. Credit cards will not be charged until you have approved of all layouts and the order ships.

Typeset Data Collection Sheet
Use this form if you are ordering Papergoods, to give us all the details about your show or event, such as the date, time, location, ticket price, and other information that will be used to lay out your posters, tickets, flyers, etc. Also use this form if you decide later to add Papergoods to an already open order.

Summary Order Form
Use this to place your final shirt order and re-orders, after you have closed your sales. Your order will ship only after you have approved all of your artwork, including your typeset shirt back if you have one.
